Cardiologist Lown combines autobiography with a plea for thoughtful and individual medical care of each patient. Although he draws his case illustrations from 40 years of practice and research, his is no nostalgic voice reporting from some comfortable ivory tower. He vigorously opposed the traditional prolonged bed rest for heart patients, pioneered in the establishment of cardiac care units, invented direct current cardioversion, which has saved many lives, and made other valuable contributions. He was one of the first Western physicians to visit China and later benefited from acupuncture treatments. In 1985 he and a Russian physician, cofounders of Physicians Against Nuclear War, received the Nobel Peace Prize. Healing, he asserts, has been replaced in his time by treating, caring by managing, and the art of listening by technological procedures. He points out that malpractice cases are not the cause of what ails U.S. medicine but are instead a symptom of the real cause--failure in the art of healing. William Beatty

The engaging memoirs of a distinguished physician who uses human interest stories to get across his message that healing the patient must once again be the focus of medicine. A cardiologist at Brigham and Women's Hospital in Boston and a co-founder of Physicians Against Nuclear War, Lown has some 45 years of experience in doctor-patient relations. In his view the crisis in health care today isn't about ballooning costs or malpractice suits but about the fact that ``medicine has lost its way if not its soul.'' His central thesis is that the medical profession has been losing its focus on healing, due in part to a romance with high technology. Lown, whose career has encompassed both research and clinical practice, acknowledges wryly that his own cardiological research has facilitated what he most deplores: the advance of technology and with it the depersonalization of medicine. Dr. Samuel A. Levine, Lown's mentor, appears in many of the stories here, for it was he who early on shaped Lown's ideas of what doctoring was all about. Stories featuring Levine demonstrate how the best diagnosticians combine the science of history-taking with the art of listening to the patient, and it is Levine who shows how the words a physician chooses can have a powerful impact on a patient's well-being. Lown, who has passed his 70th birthday, writes with compassion about the challenges of caring for the elderly, and his essay on death and dying should be required reading for all would-be doctors. This is an incredible book. Written by the cardiologist who helped bring us DC cardioversion, and lidocaine for arrhythmias, it surprisingly focuses on the human aspects of medicine.
He makes several critical points: the most important task for us as physicians is to first listen to our patients. He encourages us to make our diagnoses and prognoses on the basis of careful history taking and examination, rather than by laboratory testing. He also exhorts us to take the responsibility of appropriately reassuring and encouraging our patients, rather than just always hedging our bets to cover our malpractice worries.
The book is very well written and edited, and contains dozens of fascinating doctor/patient vignettes. He explores the boundaries of the patient/doctor relationship, and is well aware of the times it leads him astray.
He is not afraid to interject his personal religious philosophy (as an agnostic from a Jewish background) but respects those with other opinions.
This text contains far more valuable information for physicians than any biochemistry or histology text. Hopefully wise Deans will make it required reading for their medical school

Lown kritisiert zu Recht mangelnde Emphatie in unserem Gesundheitssystem, auch wenn er sich auf das Amerikanische bezieht. Jedoch sind die Probleme auch bei uns in Deutschland zu finden. Natürlich ist die Forderung, mehr Zeit mit dem Patienten zu verbringen, eine Beziehung aufzubauen und nicht nur die Apparate-Medizin zu bemühen, vollkommen legitim. Jedoch Utopie. Es fällt auf, daß viele Patientenbeispiele aus der Upper Class sind, ehemalige Physiker, Doktoren etc. Da fliegt ein Dr Lown eben 'mal 500 km, um den Patienten zu visitieren. Oder ruft täglich eine Patientin an, um sich zu erkundigen, wie es ihr geht... Ob es das für einen normalen Arbeiter auch gemacht hätte?? Nebenbei erhält man noch einen subjektiven Bericht, wie sich die Kardiologie und vor allem die Kardioversion bzw. Defibrillation entwickelt hat (an der Lown maßgeblich beteiligt war). Also ein wenig Gesselschaftsutopie mit Medizingeschichte, interessant zu lesen und dazu geeignet, sein Verhalten Patienten gegenüber in einem kritischen Licht zu betrachten. Aber solange Technik teurer entlohnt wird als eine gründliche Anamnese und körperliche Untersuchung, werden viele Forderungen Lowns Utopie bleiben.

I loved this book. Having heard Dr. Lown's science as a
trainee, I had no real idea of his humanism, or how he feels
about his patients. A Harvard Professor who is humbled
by his patients problems when they don't fit traditional
science, and admits it in print!
Someone who obviously knows and respects his patients for
their individuality and loves them despite their fallibility.
I went to see my patients the day after finishing Dr. Lown's
book wanting to match his dedication and honored to be in the
practice of medicine.
Read it! It'll energize you, and can't but help your own
patients.
